Services shutting down during nick registration

I'm running Anope 1.8.4 with InspIRCd 1.2 on a CentOS 5.2 mediatemple host. We have email confirmations enabled on NickServ, and whenever someone registers a nickname, it causes services to shut down. The log output shows:

I've attempted to change the DontQuoteAddresses directive to no avail. Not really sure if QMail is actually installed or not, there's not any package for it or binaries, but there is a /var/qmail folder. Sendmail works, and I receive email from it with no error, I also do get the email when registering, but the services still shut down. I can restart them and confirm just fine, but they stop the next time someone registers.

Any thoughts?
